Reasonable Adjustment

If you need Reasonable Adjustments, we really want to make sure that do everything we can to make those changes for you. To do that, we need to know what changes you might need, as these will be different for every person.

These will be recorded on your records here at the practice and with your consent, shared with other teams in the NHS or social care. This will help them know what changes you might need when you see them.
