Accurx Triage is used for all patient contact with the practice to request an appointment and to assist with administrative queries. It is a quick and simple online form which can be accessed by clicking on the link above. 

Our system means that appointment requests are reviewed by a clinician enabling patients to be offered advice electronically or be offered an appointment with the appropriate clinician, within an appropriate timeframe via a self-booking link.

If you are unable to book via the self-booking link sent to you, please contact reception. Please note - same day booking links will expire on the day sent.

Our telephone lines and reception desk remain open for patient with accessibility needs, in which case a member of our reception team will complete the triage form with you over the phone or in person. 

Our reception team will no longer be directly booking your appointment without the request being triaged first.

The benefits of using Accurx Triage are:

  • Available Monday - Friday 8.00am - 10.30am, 11.30am - 1.00pm and 2.00pm - 4.30pm allowing you to complete the form at a time that suits you, rather than having to telephone at 8.00am
  • Quick and easy to access – it only takes a couple of minutes to fill out the form
  • The clinician may be able to help more quickly and easily, for example with a text message, or by referring you directly to the service you need

To Cancel an Appointment

Appointments at Chelmer Medical Partnership are at a premium and a missed appointment could have been used by another patient.

If you need to cancel or change any appointment, you can:

  • Call the practice and choose the dedicated cancellation option 4
  • Attend the practice in person to cancel 
  • Complete the cancel an appointment form here

Chelmer Medical Partnership has sadly had an alarming number of DNA’s reported for February 2025, 339 patients failed to attend their appointment. This averages approximately 17 appointments per day. 

To Register for SystmOnline

SystmOnline is used for requesting repeat medication and viewing test results. You cannot book appointments via SystmOnline.

Please attend the surgery in person with photo ID to obtain a username and password. Once you have received your password please sign in within 24 hours. The login page for SystmOnline is under the prescription tab on the homescreen.

13 -15 year olds cannot have online access at Chelmer Medical Partnership.

 

Home Visits

A clinician can only visit you at home if you are bed bound and they deem that your medical condition requires a home visit. The clinician will decide how urgently a visit is needed.

To request a home visit please complete the Accurx Triage form at the top of the page. The form can be completed by a carer, friend or family member on the patients behalf.

Enhanced Access Service

GP Practices in your local area are working together as part of a Primary Care Network.

With our Enhanced Access Service patients can access appointments in the evening 18:30 – 20:00 or on a Saturday 09:00 – 17:00.

These appointments are a mix of face-to-face and telephone appointments with a range of healthcare professionals. To book an appointment complete the Accurx Triage form at the top of the page.

We are unable to deal with any other queries during these hours.

Referrals

Once a referral has been made by the clinician, the patient must communicate with the health care provider to whom they have been referred to with any queries.
