Accurx Triage is used for all patient contact with the practice to request an appointment and to assist with administrative queries. It is a quick and simple online form which can be accessed by clicking on the link above.
Our new system means that appointment requests are reviewed by a clinician enabling patients to be offered advice electronically or be offered an appointment with the appropriate clinician, within an appropriate timeframe via a self-booking link.
If you are unable to book via the self-booking link sent to you, please contact reception. Please note - same day booking links will expire on the day sent.
Our telephone lines and reception desk remain open for patient with accessibility needs, in which case a member of our reception team will complete the triage form with you over the phone or in person.
Our reception team will no longer be directly booking your appointment without the request being triaged first.

SystmOnline is used for requesting repeat medication and viewing test results. You cannot book appointments via SystmOnline.
Please attend the surgery in person with photo ID to obtain a username and password. Once you have received your password please sign in within 24 hours. The login page for SystmOnline is under the prescription tab on the homescreen.
13 -15 year olds cannot have online access at Chelmer Medical Partnership.
A clinician can only visit you at home if you are bed bound and they deem that your medical condition requires a home visit. The clinician will decide how urgently a visit is needed.
To request a home visit please complete the Accurx Triage form at the top of the page. The form can be completed by a carer, friend or family member on the patients behalf.
GP Practices in your local area are working together as part of a Primary Care Network.
With our Enhanced Access Service patients can access appointments in the evening 18:30 – 20:00 or on a Saturday 09:00 – 17:00.
These appointments are a mix of face-to-face and telephone appointments with a range of healthcare professionals. To book an appointment complete the Accurx Triage form at the top of the page.
We are unable to deal with any other queries during these hours.
Once a referral has been made by the clinician, the patient must communicate with the health care provider to whom they have been referred to with any queries.
